What we provide
Every service is ordered by the patient's physician and delivered by licensed clinicians. Most patients receive two or three of these together.
Skilled nursing
Registered nurses and LVNs for medication management, IV therapy, disease teaching and post-hospital monitoring — under your physician's plan of care.
About skilled nursingPhysical therapy
Licensed physical therapists who restore strength, balance and mobility after surgery, stroke, a fall or a hospital stay — in the home where it will be used.
About physical therapyOccupational therapy
Occupational therapists who rebuild the daily skills that keep a person independent: dressing, bathing, cooking, and moving safely around their own home.
About occupational therapySpeech therapy
Speech-language pathologists for swallowing safety, speech and language after stroke, and cognitive-communication support for patients with dementia or brain injury.
About speech therapyMedical social services
Medical social workers who help families navigate the system: community resources, caregiver support, advance directives, and the practical problems that get in the way of recovery.
About medical social servicesHome health aide
Certified home health aides who assist with bathing, grooming, dressing and mobility — supervised by a registered nurse as part of the skilled plan of care.
About home health aideFor families
You are usually calling from a hospital hallway or the kitchen table, trying to work out how a parent will manage at home. Here is what we need from you: the patient's name, the physician who is treating them, and where they live. We take it from there — insurance, orders, scheduling, and the first nurse visit.
Care is planned around the household, not the other way round. Visits are scheduled to fit work hours and other appointments, and the nurse teaches whoever will be helping at home.
Request a call backFor physicians and discharge planners
Fax orders and face-to-face documentation to (310) 507-9989. Intake confirms receipt by phone, verifies eligibility, and schedules the start of care — usually within 48 hours. You receive the plan of care for signature, 60-day summaries, and a call whenever a change in condition needs your attention.
We accept Medicare, Medi-Cal, Medicare Advantage and most commercial plans.
Referral detailsCare in your language
For many families in the foothills and the Valley, care in their own language is the difference between confusion and confidence. Our team delivers care, teaching and family updates in English, Spanish, Armenian, Russian, Tagalog, Hebrew and Farsi.

Common questions
Is home health the same as a caregiver or 'home care'?
No. Home health is skilled medical care — nursing and therapy — ordered by a physician and covered by Medicare. Non-medical home care (hourly companions, help around the house) is a different service that Medicare does not cover. We provide home health.
Who qualifies for Medicare home health?
A patient who is under a physician's care, needs skilled nursing or therapy on an intermittent basis, and is homebound — leaving home takes considerable effort and happens infrequently. We check eligibility for you before the first visit.
How much does it cost?
For eligible Medicare beneficiaries, home health is covered in full with no deductible or copay. We also work with Medi-Cal, Medicare Advantage and other insurers, and offer direct per-visit billing when coverage is unavailable. We verify benefits before care starts.
How fast can you start?
Once we receive the physician's order we schedule the start-of-care visit, usually within 48 hours and often the next day for hospital discharges.
Which areas do you cover?
Los Angeles County. Our office is in Sunland, so the foothills, the Crescenta Valley, Glendale, Burbank, Pasadena and the San Fernando Valley are closest, but we accept patients county-wide.
